UCLA Neuromodulation Referral

Thank you for your interest in our service.

We will contact your patient as soon as possible to discuss possible treatment with our service. In our initial phone call with your patient, we will explain the process of neuromodulation treatment and perform an initial screening for any contraindications to treatment.

After this initial screening, we will schedule an evaluation as appropriate. The evaluating physician will then contact you to discuss your patient's case and the recommended course of action.
